What is the study about?

Stroke survivors fall more frequently than those who have not had a stroke (73% fall at least once in the year after their stroke). There are no evidence-based interventions addressing this problem. A multifactorial falls prevention action plan (called Action Falls), which has been shown to be both effective and cost effective in reducing falls in care home residents, has been adapted for stroke survivors' use in an individual's home setting. This trial will assess whether it can reduce falls in this population.

Who can take part in the study?

People aged over 18 years admitted to hospital following a stroke who have been discharged to home.

What is being tested in the study?

Usual care plus Action Falls program. The Action Falls program will be delivered a maximum of three times – once at baseline, once at 6 months post baseline and once if the participant has either had a fall or has moved residence.

What is this being compared to?

Usual care alone

What are we trying to find out?

We are trying to find out whether the addition of the Action Falls program to usual care is better than usual care alone in reducing the number of falls in stroke survivors in the first year after discharge from hospital.
